You will not be registered until you have been into the surgery with and completed a GMS1 form, valid ID and proof of address.

When joining the practice, all patients will need to complete a registration form. Registration forms are available from reception or you can download them by following the link belowPreRegistration.

You will be required to attend the surgery to return your registration forms, can you also bring a form of ID and proof of address so that we can ensure we have the correct details registered on our system.  You will not be registered with the practice until you have signed the form.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
